§ US-based board-certified or board-eligible cardiothoracic surgeons and cardiologists § Have implanted at least 2 ventricular assist devices (VADs) in the past year § Familiar with Thoratec’s HeartMate II Device § Have a significant amount of practice devoted to late-stage heart failure patients
